Uncovering Sources of Heterogeneity in the Effects of Maternal Smoking on Infants’ Health at Birth

Abstract
Smoking during pregnancy poses significant risks to infants’ health at birth, but individual drivers of effect sizes are largely unknown. This study is the first to uncover the heterogeneity in its impact on birth weight and other health measures, using a novel machine learning based effect decomposition. In a large US sample, older maternal age increases smoking’s negative effects, while high pre-pregnancy BMI and excessive weight gain can reduce its impact on birth weight but have mixed effects on other health outcomes. These insights can improve targeted smoking cessation for pregnant women, potentially reducing healthcare costs and enhancing newborn health.
