In industries characterized by intense competition and the associated constant pressure to come up with new designs for products and services, shorten processes and lower costs, it is very frequently the case that a high division of labor develops between service providers, which is accompanied by specialization, the standardization of service components and extensive networking. In order to achieve or maintain competitive advantages many enterprises have therefore chosen to eliminate the classic company boundaries and to form collaborative business networks (e.g. automotive industry). Up to now, the health care sector has only seen the beginnings of this development. In Switzerland, like in many other industrialized countries, it is still marked by monolithic structures with low service-orientation. The fact that health care differs in structure from most other sectors is attributable to the high level of regulation which can hinder or prevent innovation, the high proportion of government investment and the associated low pressure in respect of effectiveness and efficiency, the lack of orientation towards patient benefits and widely differing interests of the individual players. Due to distinctive characteristics of different health care systems, a centralized approach to build a business collaboration infrastructure, as realized by the NHS in the United Kingdom, is not feasible in every case. Therefore approaches need to be developed which enable accessibility of services on different competency levels and for different players. Looking at the situation in Switzerland, only fragments of such a platform exist: standalone service components without strategic and/or organizational nexus of the health care provider's reality. In order to achieve best possible impact a common strategy and agreements (e.g. eHealth strategy at national, regional and local level), reference processes (e.g. clinical pathways, common administrative work practices), standardized business services components (e.g. electronic health records, insurance coverage check, e-prescription etc.) and standardized technical services components (e.g., master-patient index, health professional cards etc.) are needed. The presentation will include a brief introduction why service-orientation in health care is needed and why it is so hard to get the things right, a description of the elements of a business collaboration infrastructure for the health care sector and of how to do the right things, an experience report of different Swiss eHealth-projects which intend to establish such a platform and what pitfalls they expect.
